When running a custom web form (either Custom Data Form or Barcode Web Form), the form properly displays on the core server. However, on remote consoles, the fields do not show up.
Seemed to be a DNS issue. When the fully qualified domain name (FQDN) was used in the URL, the form appeared correctly.
The web location of the forms is pulled from the database. To update the database to use the FQDN instead of the server name in the URL run the following SQL statement, replacing with the FQDN of the server: UPDATE MP_Configuration SET Value = 'http://remote' WHERE Name = 'ERP_URL'
