How to send different surveys based on Incident field values

    Customer would like to send different surveys based on Incident field values such as Services or Teams, ie. Facilities


    Resolution

     

    In Configure Application, click on Surveys
    Create a new Survey called Facilities Survey
    Fill out questions 
    Save
    Click on Global Constants
    Create a new Global Constant called FacitilitesSurvey
    Enter "Facilities Survey" in the Value
    Save
    Click on Business Objects
    Open Incident
    Click on the Workflow tab
    Edit the Incident Survey workflow
    Drag an IF block to workflow grid
    Edit the IF block by giving a title of Facilities
    Add the field that will contain the value. ie. Service equals to Facilities
    Edit the existing Notification block
    Screenshot the From, To, and Subject line
    In the Body, click on Edit Source and copy the entire contents
    Cancel
    Drag and drop a new Notification block
    Edit the block and fill out the From, To and Subject line
    In the Body, paste the copied source code from the existing notification block
    Within the <A> tags, make the following changes as shown in the before and after below:

    Save the Notification block
    Remove all of the connectors between the blocks
    Drag the Start to the IF block
    From the True on the IF block, connect to the new Facilities notification
    From the False on the IF block, connect to the existing default notification
    From both Notification blocks, connect to the Stop block
    Save
    Click on the green arrow 
    Publish the Workflow
