You will need to make sure that the status you are going to in the Request workflow does have the "Add Incident" and "Create Incident" in there. Without that, you will get the error.

Null value set for attribute CreationUser of class type RequestManagement.Assignment. This attribute is mandatory and must contain a non-null value.
I'm not having much luck at this - process design is still relatively new to me.
This typically means that a value that needs to be populated out like "Creation User" is not being populated. This could be because of a value type that is incorrect.
Along the lines of what Jaimie said, check your Request process for your initial assignment... are you referencing a value there for the CreationUser? I would think that this value is getting filled automatically with the value for CurrentUser.
Did you manage to resolve this issue I am also getting the 2nd error message when I try to put this process in action. I have filled in the creation user with current user on the request window which is being called in the process but I am still getting the error.
