It sounds like you have configured it ok - if you try running it within the Crystal reports component will it run? You will be prompted for guid value when you run it from there. If you can extract one from the rm_request table to paste in when you are running the report.
This might be a good one to raise with support so they can talk you through this process and other troubleshooting steps a bit quicker.
Have you confirmed the report file actually exists? I believe that 7.4 (if that's your version) didn't come with an OOTB Request report, though I could be wrong on that, so double-check that the report file is actually in that location and that the path and filename is exactly correct.
Also, as it is printing it usually looks in the default reports path for the file, so check that the Request.rpt file isn't in a sub-folder or something.
I am getting the same error message
"The following report failed to load: \\Servername\SQL\Request.rpt"
I don't get that from printing from incident. It appears to be missing the "Request.rpt" file.
