Any chance you have the Submit Action available on the the status itself, as a Optional Action Instance?
Goto your process in Process Designer, highlight the Open status, select properties from the shortcut on the left(Not by right clicking), select (collection) from the Option Action Instance, and see if the action is listed as a Selected Action Instance. If so, remove it from here, as it wouldnt move your process like your design indicates you want.
I've seen the error before while creating a new process, it just isn't fully sparking my noggin of what causes it. I know that what i listed above can cause the issue though.
Thank you, unfortunately I had checked this previously and there is only 1 submit action at Open, I am a little confused.
This message is sometimes displayed if you add an item to the process delete it and then re-add the same item again. The application is not checking the order in which the items were added to the process when applying the changes so it is actually trying to do - Add Item --> Add Item -->Delete item.
You will have to close the process without saving the changes and then make the changes again. To avoid this issue in future; if you have added an item to the process e.g Add Note and you subesquently delete it, make sure you save the process before attempting to re-add the item to the process.
Thanks Martyn. I am trying to avoid that. I have done quite a bit of work, saving as regularly as I can but I don't want to lose the work.
Has this been fixed in a later release as it is unacceptable to have to recreate the process from scratch because of this problem.

Problem 5288 - "Cannot save process as there are multiple x actions on the x status" error when modifying a process.
This is a known issue, unfortunately only thing you can do is revert to last save point and recreate.
