This will occur if you have the wrong Bundle ID or Bundle Version referenced in the software payload. I would recommend you install the app manually on a device, then perform a sync then validate what these values should be in the device details under Installed Software. The Manufacturer column will be the Bundle ID and the Revision should be the Bundle ID.
