you could add a new user object to your note object and then assign the email address to the user-objects email-address attribute. So LDSD select automatically the correct user and saved it to the note.
So you have the approver in a separate object-attribute on the note object.

Hi, have you gotten this to work. We just did this and I now have the same issue.
In the "Notes" form, there should be a Raise User field which would be the person the email was sent from. The created by would be System because it technically was created by the system.
What I've seen in the past is create a new user attribute on the Request object and call it "Approved By". Add it to your form. Then, in your process, after your Approve action, but another auto action with association to a window as true and set a value type of Notes.RaiseUser in there. This would set the Approving Person to be the one who sent the approval via email.
Also, what version are you using?
I am using 2017.1. I will look at what you stated and see if I can figure it out with those directions. It makes sense to me but I am not very good yet when it comes to adding objects and things so I will give it a shot and let you know.
