Check the default query for the Request Management\Attachment business object. (it should be Request - Attachments)
That is what defines that tab at the bottom of the window.
Your screenshot shows that there are 4 pages of results, perhaps there are no attributes shown on the query.
Attributes on my query are: Title, Created By.Title, and Creation Date.

On the mentioned Business object, the "request-attachments" query is being used:

It seems that the attachment is being added, but doesn't appear visible.

Open that query in query and report designer and check the attributes and criteria.
As shown on the second screenshot, the query "Request - Attachments" has title, created by and creation date as attributes. No criteria set on the query so, with the setup, it should be showing the attachment (Correct me if i am wrong).

If you run the query, are there values in all the fields? No blanks?
from what I can see, this is not an Incident. You mixed up two things here.
One is the Attribute "Technical Documentation" you created on your request object with the type "attachment". When you save a document there, it is saved to that attribute.
In your request window you have the attachment collection but this collection is filled by an "add attachment" action in your request process. You will never see your technical document there. You have to add this attribute "technical document" to the second window in your first post.
