We had an issue with one of our customers regarding LDAV because clients were not able to update to the newest pattern even when downloading from source was the only possible way to get the patterns. Preferred Server and peer download was deactivated but no chance. The core says up to date, but the patterns on the client were not. We spend hours of troubleshooting because some clients were able to update from the source. Our final solution now is to use the Kaspersky Update Utility 3.0.
We scheduled a windows task on the core to run the downloader utility every x hours and download them to a temporary folder. Then we copy everything to “C:\Program Files\LANDesk\ManagementSuite\ldlogon\antivirus8\Win\basesEP10” and overwrite if needed. Then we synchronize this folder to the preferred servers and now everything works as expected. The clients are up to date since one week now.
It’s a bit “poor” that we have to create such a workaround and the acceptance for the LANDESK Antivirus is going nearly to zero because of many month of pain with the outdated pattern.

Also noticed it crashing when running /updatefrominternet , have been able to work around it by running it from an elevated command prompt

I have the same issue but only for a handful of client computers. I've noticed that on all of the affected computers that the LANDesk Agent Settings section in inventory is missing. This is how I was able to create a query to find the affected clients. Nothing I do is able to resolve this issue. Another symptom is that you can Exit AV by right clicking the KAV icon in the system tray. Branding is off as well. When doing a vulscan our logo is missing. Vulscan also always has a "failed to update antivirus settings" message or something like that. I've tried doing a full agent uninstall and reinstall. We've tried deleting the computer from the landesk console, we've tried all kinds of fixes in the community with no luck at all.
Can you check your inventory to see if your agent settings are missing as well? The below screenshot shows it when it exists. Look for it not being there.
