If you have not already, give support a call and get a case created. This might be a defect but we will need to investigate further.
Is that happening when you run the "Upgrade" of DB in Configuration Center?
What version are you upgrading from as I'm wondering if you are trying to upgrade to 2016.4 from pre 7.8.0 starting DB?
If that is the case you need to upgrade via a 7.8.0 interim version first.
