Cascading validations should allow you to accomplish this. You can also have it auto-select a value if there is only one.
If you set your 'data' field to be the field holding the email address, you can reference that field directly in your notifications as well. $(T
Sub-category is a good example to go from.
You can have each field set as a constraint.
You would then build your table much like what you already have laid out.

We had deconstructed the Service, Category, Subcategory forms before I came on board so I did not every see it functioning. It makes sense!
