On the workflow click the configuration button, the one where you set the trigger, there is another tab there which will send an alert if the workflow fails.

Thank you for the information, I apologize I should have clarified a bit. I need to send a failure notification from the Request Offering Workflow. On the same lines as you described above but I do not see anywhere to put the Exception Notification in a Request Offering Workflow.
here is where mine is
Then you'll click on the pencil to open and edit
you'll customize the email that you want sen and to whom > then click save on the workflow block > then save at the top
once done you'll go back to the list of versions and you will see a icon looks like a green Christmas tree where my pink circle is (I don't have that icon since I don't have a new one to promote)
youll click the green Christmas tree looking icon and it will ask you if you are sure you want to promote. if yes ok then you should be good (it will only take effect on the new SRs that are submitted after this is promoted)

There isn't an exception as such on RO workflows as JenBouillion points out you can do something with the fulfil request workflow but that is similar the the above.
Some workflow blocks have a fail option:
You could run a notification from this upon failure. Things like task blocks etc. will just fail to create and have no alert option, you could call the task creation from a Quick Action block which has the fail exit point to alert on.
